New Patient Paperwork: Complete in Office or Bring it With You
Complete in Office – In making your initial appointment, we will generally ask that you arrive 30 minutes prior to your scheduled appointment time to complete new patient registration forms, provided on a clipboard as you arrive.

Bring it With You – To speed up the registration process, you are encouraged to print & complete forms above. Please arrive 15 minutes (vs. 30 minutes) prior to your scheduled appointment time. This allows us to ensure you, and any insurance information, are properly set-up in our system.

We strive to explain any initial questions you have prior to your first appointment.
Don’t hesitate to contact us if any additional questions come up once your initial appointment time is made!
It is especially important to LET US KNOW if you:
- Are pregnant or may be pregnant
- Have any serious health concerns
- If you’ve had dental x-rays taken recently that may be sent to Comfort Dental prior to your initial visit
