FOR YOUR FIRST APPOINTMENT:
We strive to explain any initial questions you have prior to your first appointment.
When you make your initial appointment, we ask that you arrive 30 minutes prior to your scheduled appointment time to complete new patient registration forms.
OR
To speed up the registration process, you are encouraged to print and complete the following forms & either:
- Bring forms with you to your first appointment
- Fax forms to 765.742.0152 before your appointment
If your forms are completed and/or faxed, please arrive 15 minutes prior to your scheduled appointment time so we can ensure you are properly set-up in our system.
