YOUR INITIAL VISIT:

We strive to explain any initial questions you have prior to your first appointment. Don’t hesitate to contact us if any additional questions come up once your initial appointment time is made!

It is especially important to let us know if you: are pregnant, have any serious health concerns, or if you have had dental x-rays taken recently that may be sent over to our office prior to your initial visit.

WE need “new patient paperwork” completed – YOU have options:

OPTION 1 – In making your initial appointment, we will generally ask that you arrive 30 minutes prior to your scheduled appointment time to complete new patient registration forms, provided on a clipboard as you arrive.

OPTION 2 -To speed up the registration process, you are encouraged to print & complete forms below & either:

Bring completed forms with you to your first appointment
Fax completed forms to 765.742.0152 before your appointment

If you choose OPTION 2, please arrive 15 minutes prior to your scheduled appointment time. This allows us to ensure you, and potentially your insurance information, are properly set-up in our system.
